For the purposes of the provisions of Regulation (EU) 2016/679 of the European Parliament and of the Council, of 27 April (General Data Protection Regulation), the commercial company Mental Health Clinic Isabel Henriques, Lda, with NIPC 516480081 put HOLLAND company information and head office at , email address admin@isabelhenrique.com and web address www.isabelhenriques.com, informs the holder of the personal data (Client) that:

1º) The entity responsible for the processing of the personal data is the commercial company Mental Health Clinic Isabel Henriques, Lda and its franchisees.
The personal data to be processed are the following: name, address, e-mail address.

2º) The Client's personal data is processed because it is necessary for commercial quotation, conclusion and execution of the contract or because it has been the object of consent.
The omission or inaccuracy of such data or other information provided by the Customer is of his sole responsibility.

3º) The personal data provided by the Client shall be processed and stored by computer, and shall be used by Mental Health Clinic Isabel Henriques, Lda and its franchisees for: marketing and sales, client management and service provision, accounting, tax and administrative management, litigation management, network and systems management, information security and tax security control, and compliance with legal obligations.

4º) You may oppose the processing of data at any time and by any means, without prejudice to compliance with the obligations imposed by law.

5º) Personal data shall be kept for different periods of time, according to the purpose for which they are intended and taking into account legal criteria of necessity and minimization of time of conservation, without prejudice to the periods legally defined for the various purposes.

6º) You have the right to request information from Mental Health Clinic Isabel Henriques, Lda and its franchisees about the type of treatment your data is being subjected to. The information shall be provided in writing and the request should be made through the following email geral@mentalhealthclinicisabelhenriques.com. If the Client so requests, the information may be provided orally, as long as the identity of the holder is proven by other means.
7º) Your personal data is provided to Mental Health Clinic Isabel Henriques, Lda and its franchisees for accounting, tax, marketing and sales purposes. Mental Health Clinic Isabel Henriques, Lda and its franchises may use subcontractors for the processing of personal data, namely for client management, service provision, billing and litigation management, being these entities obliged to develop the adequate technical and organisational measures.

8º) You have the right to access a copy of the personal data being processed. If the request is submitted by electronic means, the information shall be provided in a commonly used electronic format.

9º) You have the right to request the rectification of your personal data at any time.

10º) You have the right to request the erasure of your personal data, without prejudice to the fulfilment of legal obligations.

11º) You have the right to oppose the erasure of your personal data and to request the restriction of their processing, without prejudice to the fulfilment of legal obligations.

12º) You have the right to oppose the use of your data for direct marketing purposes.
13º) Your personal data shall be processed automatically or non-automatically.

14º) You have the right to request that your personal data is transferred to another entity.

15º) Your personal data are protected by ensuring that pseudonymisation is carried out and that the ability to ensure the ongoing confidentiality, integrity, availability and resilience of the processing systems and services is maintained, the ability to restore availability and access to personal data in a timely manner in the event of a physical or technical incident, and a process for regularly testing, assessing and evaluating the effectiveness of technical and organisational measures to ensure security of processing.
